玉牌为和阗白玉质地，白润细腻，致密温润。器型为扁平长方形。牌首镂空雕饰一对卷草夔龙纹。两侧、底角及底部正中皆透雕小孔。牌面作“亚”字形开光，正面减地浮雕一坐一立两童子，手捧灵芝和宝瓶，喜笑颜开，神态生动，雕工传神。背面正中阳刻“平安如意”四字篆文印章款。整器打磨细致，寓意吉祥，为清代玉牌佳品。
