长11.6cm；宽8cm；厚1.6cm
|和田青白玉制成，玉质细润，选料大气，体作扁平长方圆角形，正面以多层镂雕法饰松下神仙故事图，背面平而无饰，仅留镂琢的孔洞。作品所见是松林中局部景色，以人物为主体，松树、灵芝、仙鹤、浮云为背景，精琢细镂，惟妙惟肖。关于此器的年代说法不一，从其中心人物的冠饰上看具备五代的时代特征，从构图及对局部景物细致描写的特点上看又应是宋代制玉的特点。
